Forms

Forms must be completed in blue or black ink. 

Completed and signed forms may be emailed to the Registrar's Office at transferevaluation@cofc.edu.  Forms will only be accepted via email if sent from the student's college-issued email address (username@g.cofc.edu). 

Forms can be fully completed, signed, and submitted electronically.  To use the "fill and sign" features built into the form, you must download it to a computer and complete it using Adobe Acrobat.  Completing the form in a web browser will not enable the electronic signature feature.  Forms will not be processed until they are signed (Adobe e-signature or physically signed).  

The forms listed below are for CURRENTLY ENROLLED COLLEGE OF CHARLESTON UNDERGRADUATE STUDENTS ONLY.

    Hours Earned + Currently Enrolled Hours + Requested Hours must = 89 Hours or fewer

    Rising seniors (>= 90 hours) must complete the Senior Year Petition

    A degree candidate at the College of Charleston who wishes to receive College of Charleston credit for courses at another institution must submit the Coursework Elsewhere form. Official permission to take coursework elsewhere is reflected via DegreeWorks. If you are required to submit proof of permission (transient permission) in order to register at another institution as a transient student, submit the Transient Permission letter and attach a printed copy of your approved transfer credit from Degree Works.   • Coursework Elsewhere During Senior Year Petition - FOR COURSES TAKEN DURING THE FALL 2020 SEMESTER FORWARD

    Hours Earned + Currently Enrolled Hours + Requested Hours >= 90 Hours

    This form is used by seniors who want to take a course or courses during their senior year away from the College of Charleston. The form must be signed by the department chair or program director of the student’s major before it is submitted to the Registrar's Office.  Up to twelve (12) credit hours may be approved. If thirteen (13) to eighteen (18) credit hours are requested, the student must attach justification in a separate letter and a copy of their Unofficial Transcript from MyPortal, all of which will be forwarded to the Faculty Committee on Academic Standards (FCAS) for approval.  If more than eighteen (18) credit hours are requested, all documentation will be forwarded to the Provost or their designee for approval.

  • Transient Permission Form - FOR COURSES TAKEN DURING THE FALL 2020 SEMESTER FORWARD
    This form is now part of the Coursework Elsewhere form and should be sent to any institution that requires College of Charleston approval before the student can take a course as a visiting student.  The College of Charleston does not sign other institution’s transient permission forms. In lieu of a signature on another institution’s form, the Registrar's Office provides students with a signed CofC Transient Permission Form and a “VP” (visiting permission) indicator on a student’s Degree Works degree audit as permission to take a course at another institution.

Decline and Reinstate Credit

  • Decline Credit Form
    This request to decline credit awarded by Advanced Placement (AP), International Baccalaureate (IB), Cambridge Assessment exams, Military credit, and/or transfer credit must be approved by a student's academic advisor in the Academic Advising and Planning Center or his/her faculty advisor in the major department (for declared students).  It should be submitted to the Registrar's Office before courses are retaken.

  • Reinstate Credit Form
    This request to reinstate credit awarded by Advanced Placement (AP), International Baccalaureate (IB), Cambridge Assessment exams, Military credit, and/or transfer credit must be approved by a student's academic advisor.  Students who previously declined exam credit and are currently enrolled in a course to earn the same credit, must return this form to the Registrar's Office prior to the withdrawal deadline of that academic term to reinstate their exam credit.
